This report must be completed by a CCCDI licensed plumber.
If there is electrical work at the job site, this worksheet must be submitted prior to permit issuance.
For a fence permit, a permit application and this worksheet must be completed and submitted with 3 copies of your Plat of Survey.
For a garage permit, please complete the garage worksheet. Make 3 copies and attach a current Plat of Survey to each copy, making sure to show the location of the proposed garage. Submit the 3 sets along with with a completed permit application, and a completed electrical worksheet to the Building Office.
If there is mechanical work at the job site, this worksheet must be submitted prior to permit issuance.
For an interior remodeling permit, this worksheet must be submitted along with the permit application for remodeling/renovation.
If there is plumbing work at the job site, this worksheet must be submitted prior to permit issuance.
For a roofing permit, a permit application and this worksheet must be completed and submitted.
This worksheet will be used to determine if storm water detention is required for the proposed work. It is required for all new commercial and residential (larger than 3 units) buildings and for major additions/ rehabilitations to existing buildings.
For a tuckpointing permit, a permit application and this worksheet must be completed and submitted.

This worksheet is utilized to calculate the change in plumbing fixture units based on the new work to determine whether the existing water service is adequate or requires an increase in size.

One and two family residential and commercial and multi-family residential window replacement require a building permit.
Owner Affidavit Forms:
For Single Family residences that are 100% owner occupied (no rentals), the owner of the property may act as their own general contractor provided they currently reside at the premises. "Owner Affidavit Forms" (Electrical, Licensed Plumbing, & Mechanical) must be completed and accompany the Corresponding worksheet above (i.e. Plumbing Worksheet and Plumbing Owner Affidavit) must be completed together prior to permit issuance.
