Budget Forms for Mental Health Board’s Fall 2009 Funding Proposals
In order to facilitate the Fall 2009 funding allocation process, the Evanston Mental Health Board is providing you with computerized budget forms to complete your funding application. Most of the forms have been developed in Microsoft Excel; Form 1C was developed in Word. Each form is a separate file. Where appropriate, there are formulas in each form to make some of the necessary calculations for you.
These files will enable you to fill out the forms on your computer. Please keep in mind that you are responsible for all of the information that you submit in your application. Please double-check the results of any calculations that are made on these forms to ensure that they are accurate. If you find a problem with any of the forms or calculations, please let us know right away so we can fix it and alert others who are also using these forms.
