Complaint Steps 
To Register a Complaint
- Submit the Complaint Form, providing all available information.
- Await a response from the department's Office of Professional Standards (OPS).
Steps in the Complaint Process
- OPS may route the complaint for additional follow-up and/or investigation within the department.
- The following additional steps may be required, based upon the seriousness of the complaint and/or your desire to file a formal complaint:
a. Complete a sworn written complaint form (required under Public Act 93-0592).
b. Complete a taped interview with OPS staff detailing your complaint.
c. View photographs of department members (if necessary to identify the accused member).
d. Provide names/contact information for any witnesses to the incident complained of.
e. Provide medical records related to the complaint.
Please note:
- The department accepts all complaints.
- Investigating a complaint takes time.
- In the event of dissatisfaction with disposition of your complaint, you may appear before the City's Human Services committee to discuss your complaint.
- OPS investigations/dispositions in no way influence pending court matters.
