To Request to View Files / Freedom of Information Act Request
Evanston Fire Department occupancy files may be viewed at the Evanston Fire & Life Safety Headquarters located at 909 Lake St., Evanston, IL by appointment only. In order to make an appointment to view the occupancy files you must submit a Freedom of Information Act Request to the City Clerk.
To Request a Copy of a Fire Report
If you need to request a copy of a fire report, please send a letter stating that you had a fire in your building/home. The charge for a fire report is $10.00 with the check made payable to the Evanston Fire & Life Safety Services. In the letter, please give an Identification Number (Driver's License, Social Security, etc.), and the reason why you wish a copy of the report, i.e. if you are a tenant or the owner of the property.
Also if you can provide a date on which the fire occurred as well as the address, it will help to speed the process along. If this is a vehicle fire which occurred in Evanston, be sure to list any information about the Vehicle which you can provide such as Owner of the vehicle, type of vehicle, and license # if these are available.
If this request is from an Insurance Company, please include in the letter your Case or File Number, along with a self-addressed envelope. Make sure the request letter is on your company letterhead.

To Request an Investigation Report
A Fire Investigators' Report on a Fire call may only be received by subpoena. In the subpoena, make sure that you list the date of the fire, as well as the address in which in the fire occurred.
The subpoena should also be sent to the Evanston Fire & Life Safety Services Headquarters, 909 Lake St., Evanston, Illinois 60201.
