Food Service Manager Certification
The City of Evanston offers online training to help Risk Category 1 and 2 facilities meet the State of Illinois Food Code requirement that they employ one or more certified managers to supervise food service operations.

For initial certification, here are the steps to follow:
- Visit the IL Dept of Public Health (IDPH) site to understand your options and responsibilities.
- Create an account on the 360training.com site.
- Start the course.
- Register for the exam date that is convenient for you and that allows enough time to complete the course by sending an email to Ellyn Golden at egolden@cityofevanston.org. Finish the course. Print the "Certificate of Completion" for your records.
- Take the exam at the Evanston Civic Center, 2100 Ridge Avenue, Evanston, IL 60201.

Contact IDPH if you have not received the results of the exam within 4 - 6 weeks by calling (217) 785-2439.
- If you passed the exam with a score of at least 75%, you will receive an application form to complete and mail with payment.
- If you do not pass the exam with the minimum score of 75%, you will receive a letter indicating that you have 6 months from the date of the initial exam to re-take the exam two more times. This can be done in Evanston at no additional fee but you must register by sending an email to Ellyn Golden at egolden@cityofevanston.org. Space is limited and it is best to register early.
To renew a valid certificate, here are the steps to follow:
- Visit the IL Dept of Public Health (IDPH) site to understand your options and responsibilities.
- Create an account on the 360training.com site and start the course. There is no exam requirement.
- Email Ellyn Golden at egolden@cityofevanston.org that you are taking the 15-hour online course to meet the continuing education requirement for renewal. You are not required to take the exam; you must, however, finish the 15-hour course before your current certificate expires.
- Call 847-866-2947 to schedule a time to come to the Health Deparment. Bring in a copy of your current certificate (that is proof of you being eligible for recertification) and verification that you have taken the 15-hour course. We will then sign the form and submit it to the state.
- Submit the renewal request form and renewal fee to the State. This is your responsibility. There are significant consequences for not meeting the deadline imposed by the State.
- Contact the State if you do not receive your renewed certificate within 6 weeks by calling (217) 785-2439.
- 2011 exam date - DEC 14

2012 exam dates
- JAN 11, FEB 8, MAR 14, APR 11, MAY 09, JUN 13,
- JUL 11, AUG 08, SEP 12, OCT 10, NOV 14 and DEC 12.
- The exam consists of 75 multiple choice questions.
- The minimum score to pass the exam is 75%.
- There is no additional cost to take the exam.
- Exams are held in Room 2403 at the Lorraine H. Morton Civic Center, 2100 Ridge Ave, Evanston, IL 60201.
- Exams begin at 9AM. You are allowed 2-1/2 hours to take the exam but many finish in less time.
