Help the Evanston Health Department Reach EPLAN Goals, Take the Survey
The Evanston Health Department conducted a survey to determine the health needs of our residents. In 2011 the department analyzed the results and developed the EPLAN (Evanston Project for the Local Assessment of Needs).
This plan will guide the Health Department in its mission to protect, preserve, and promote wellness for people who live, work, and play in Evanston through creative and sustainable partnerships. The department has partnered with Northwestern University to administer brief surveys checking on their progress reaching the goals set by the EPLAN.
This survey will be available from February 12-25, 2012. Please take a few moments to complete the survey and help us improve the health of Evanston.
