Erie-Evanston Health Center Now Enrolling Residents in CountyCare

CountyCare is available now and can pay for many of your health care services. CountyCare is a new Cook County Medicaid program for adults. It may pay for most of the services you receive at Erie. And it alsoCountyCare Erie Spanish.png covers other medical services you may receive outside of Erie such as hospital visits, medical equipment, home health services and physical therapy.

"CountyCare is a Medicaid program through the Affordable Care Act  or (Healthcare Reform). The uninsured are less likely to receive preventive care; are often diagnosed with disease in more advanced stages; and are more likely to die prematurely than individuals with health care insurance. They also are more likely to first access health care through expensive hospital emergency rooms than ordinary office visits because they wait to seek care until a health crisis is well underway. CountyCare can offset this fragmented method of care by insuring those who are currently uninsured. Here at the Evanston Health Department we are excited to  inform eligible  residents that they can sign up for CountyCare says Health," explained Evanston Health Department Director Evonda Thomas-Smith.

You, your friends and other family members may be eligible for this exciting program!

To qualify for CountyCare patients must:

• Have annual income at or below $15,282 for individuals and $20,628 for couples

• Live in Cook County

• Be 19-64 years old

• Not be eligible for other Medicaid plans

• Not be eligible for Medicare

• Be a legal immigrant for five years or more or a United States citizen

• Have a Social Security number or have applied for one
