Evanston Community Health Assessment Now Closed

The Evanston Health Department would like to thank residents who participated in the 2013 Community Health Assessment.  This year 460 people completed the completed the survey, a 40% increase from 2010. Results from this year’s survey will be used to generate two important planning instruments – a Community Health Assessment and a Community Health Improvement Plan. These documents will summarize demographic information, state and local health statistics and survey results to identify our community’s most pressing needs and create an action plan for moving forward.

Assessment information will be presented at various public meetings and the 2014 Health Summit, where residents and strategic partners will collaborate with the Health Department to prioritize activities and set community health goals. Results from the 2010 Health Survey were included in the EPLAN, or Evanston Project for the Local Assessment of Needs, which is available here.
