Have you ever wondered what would happen if you were incapacitated and unable to make healthcare or financial decisions?  Evanston community members are invited to participate in a free presentation and workshop addressing legal services for powers of attorney for healthcare and property and living will declarations on Wednesday, September 6 and Wednesday, September 20 at the Levy Center, located at 300 Dodge Ave., in Evanston.

Presentation (9/6)
The presentation will be held on Wednesday, September 6 at 1 p.m. and will address the definitions and execution process of powers of attorney for healthcare and property as well as living will declarations. The presentation will also provide the information necessary to participate in the September 20 workshop. No registration is required.

Workshop (9/20)
Attorneys from the Center for Disability and Elder Law as well as their partners will be on site on Wednesday, September 20 from 1 p.m. to 3:30 p.m. to assist older adults with completing powers of attorney for healthcare and property and living will declarations. The goal is for attendees to leave the workshop with all of the desired documents signed, notarized and ready for execution.  This is a free service but registration is required.  Please contact the Levy Center at 847-448-8250 to register or for more information.
