Passport Processing Available By Appointment
Community members can conveniently apply for and renew their U.S. passport at the Morton Civic Center by scheduling an appointment with the City Collector’s Office, the City of Evanston announced today.
Appointments can be scheduled up to four weeks in advance online at cityofevanston.org/passport or by calling/texting 847-448-4311. Passport services are offered by appointment only.
To apply for a passport in person, community members must bring the following to their appointment:
- Form DS-11
- Evidence of U.S. citizenship
- Passport photo (2x2 inches)
- Valid photo identification
- Check or money order for passport fees payable to the U.S. Department of State (No credit card or cash)
- $35 execution fee (cash, credit card, check or money order accepted)

Applicants must apply for a passport in person if they are applying for their U.S. passport for the first time; are under the age of 16 or renewing a passport issued when they were under the age of 16; are renewing a passport that was lost, stolen, or damaged; are renewing a passport issued more than 15 years ago; or are changing their passport’s sex marker. If none of these apply, community members may be eligible to renew their passport by mail using Form DS-82.
