TO APPLY: Complete and email the application below to hhslicensing@cityofevanston.org with a required drawing of the layout of the outdoor patio showing the dog designated area and disinfection protocol (please see the Diagram Sample and Protocol requirements below).
NEW Application to Allow Dogs at Outdoor Restaurant Patios
IMPORTANT INFORMATION - PLEASE READ BEFORE SUBMITTING APPLICATION:
- Dogs are not allowed at rooftops or any outdoor patios where they have to pass through the kitchen, food preparation, or storage areas.
- A layout of the outdoor patio or sidewalk café showing the dog designated area must be submitted to the Health and Human Services Department. Please refer to the sample site plan drawing for guidance.

The food establishment must follow a disinfection protocol for fecal/urine incidents and submit it to the Health and Human Services Department (EHHS) for approval. - If a sidewalk café permit is required, issuance is required before permission is granted for dogs to be allowed.
- Approval must be granted to the applicant by the Health and Human Services Department before any dog is allowed at an outdoor dining patio or sidewalk café.
