Premise Alert Program Notification Form

The Premise Alert Program (PAP) is a confidential computer-aided dispatch (CAD) database designed to enhance the safety of people with disabilities. These individuals, their families, and/or their caregivers may voluntarily submit information about their circumstances for inclusion in the PAP database free of charge.

Mandated by a law promulgated on August 28, 2009, the PAP database assists police departments by informing the responding officer of the resident's disability. This information enhances the officer’s potential for providing the highest quality service to the individual living at the location calling for service.

The Evanston Police Department’s PAP pre-dates the August 28 law mandating it. However, given the law’s recent promulgation, the department finds it opportune to remind residents that the service is available and to encourage them to use it. Be assured, that having PAP information at hand enables the department to serve residents better.


Enrollment forms are/will be available at the Evanston Police Department, the various fire stations, the Lorraine Morton Civic Center (Department of Health and Human Services), and on the city’s website
